Pain in the hip and sacroiliac (SI) joint region can present in many ways — deep groin or buttock pain, a dull ache that worsens overnight, or sharp catches with stairs and transitions. Because the hip and SI joint sit at the crossroads of the spine and lower extremities, dysfunction here often radiates pain into the back, leg, or knee.

Common contributors include SI joint dysfunction, hip impingement, muscle imbalances between the gluteal and hip flexor groups, leg length discrepancy, and pregnancy-related changes to the pelvic girdle.
